As the dental profession increasingly balances patient-centered care with environmental sustainability, a growing body of research is exploring whether silicone toothbrushes could offer advantages beyond reducing plastic waste.

A newly published review has found that silicone toothbrushes may provide a promising alternative for certain patient groups, particularly children, older adults, and individuals with reduced hand dexterity who often struggle with conventional toothbrushing techniques.

The findings, published in PLOS Global Public Health and highlighted around World Environment Day, are drawing attention not only because of their potential oral health implications but also because they raise important questions about the future design of preventive dental care products.

A closer look at silicone toothbrushes

Traditional toothbrushes typically use nylon bristles attached to plastic handles. Silicone toothbrushes, by contrast, utilize softer silicone bristles that are designed to clean teeth while potentially reducing trauma to oral tissues.

Researchers conducted a scoping review of 10 English-language studies investigating the role of silicone toothbrushes in preventive oral healthcare. The review evaluated available evidence related to plaque removal, gingival health, patient comfort, safety, and environmental impact.

Although the overall evidence base remains limited, the findings suggest that silicone toothbrushes may perform comparably to conventional toothbrushes in several key areas.

Can silicone toothbrushes remove plaque effectively?

One of the most important questions for dental professionals is whether silicone bristles can adequately remove dental plaque, the primary contributor to tooth decay and periodontal disease.

According to the review, several studies reported that silicone toothbrushes demonstrated effective plaque removal and, in some cases, performed similarly to traditional nylon-bristled toothbrushes.

Researchers noted that while the evidence does not indicate superiority over conventional brushes, the findings suggest that silicone designs may provide an acceptable alternative for specific patient populations.

The review's authors emphasized that plaque control remains the foundation of preventive dentistry, making further investigation into alternative brushing technologies particularly relevant.

Reduced risk of gum injury and tooth abrasion

Beyond plaque control, the review identified another potentially important benefit.

Which patients could benefit most?

Perhaps the most clinically significant aspect of the review involves patient groups who often face challenges with daily oral hygiene.

The authors highlighted potential benefits for:
Children developing brushing skills
Older adults with declining motor function
• Patients with arthritis or reduced hand dexterity
• Individuals with neurological conditions affecting coordination
• Patients experiencing discomfort during brushing

For these groups, toothbrush design can play an important role in determining whether effective oral hygiene routines are maintained consistently.

Researchers suggest that softer silicone bristles and simplified brush designs may improve comfort and user acceptance, although additional studies are needed to confirm these observations.

Sustainability enters the oral health conversation

The review arrives at a time when sustainability is becoming an increasingly important topic within dentistry.

The Oral Health Foundation has previously estimated that approximately 256 million toothbrushes are purchased and discarded annually in the United Kingdom alone, highlighting the significant environmental footprint associated with oral hygiene products.

One life-cycle assessment included in the review reported that toothbrushes featuring silicone bristles and polypropylene handles performed better across 18 environmental impact categories compared with toothbrushes using nylon bristles and polypropylene handles.

While environmental performance varies depending on manufacturing methods, materials, transportation, and disposal practices, the findings contribute to a broader discussion about reducing waste in preventive oral care.

Potential role in low-resource settings

Researchers also identified a possible future role for silicone toothbrushes in low-resource environments.

Some evidence suggests that certain silicone designs could offer practical advantages in settings where access to clean water is limited. However, the authors caution that this area remains largely under-researched and requires further investigation before any firm conclusions can be drawn.

Improving access to effective oral hygiene tools remains a major public health challenge in many parts of the world, making innovations in toothbrush design particularly relevant.

What are the limitations of the evidence?

Despite the encouraging findings, researchers stress that the current evidence should be interpreted cautiously.

The review included a diverse mix of studies, including clinical investigations, laboratory research, an animal study, and a life-cycle assessment. Importantly, the review did not conduct a formal risk-of-bias assessment.

As a result, experts say there is currently insufficient evidence to recommend a widespread replacement of conventional toothbrushes with silicone alternatives.

Further high-quality clinical trials are needed to evaluate:

• Patient acceptance and compliance
• Cost-effectiveness
• Real-world implementation feasibility

What this means for dental professionals

For now, the review does not support a blanket shift away from traditional toothbrushes.

However, it does encourage dental professionals to consider whether silicone toothbrushes might be appropriate for individual patients who struggle with conventional brushing because of dexterity limitations, discomfort, oral sensitivity, or other barriers to effective plaque control.

As dentistry continues to evolve toward more personalized and preventive approaches, toothbrush design itself may become an increasingly important component of oral healthcare delivery.

The emerging evidence suggests that silicone toothbrushes could eventually play a role in supporting both patient-centered care and sustainability goals. Whether they become a mainstream alternative will depend on the results of future research, but the conversation surrounding innovative oral hygiene tools is clearly gaining momentum.
