Sugar substitutes have long been promoted as a healthier alternative to sugar. They do not feed cavity-causing bacteria the way sugar does, and some — such as xylitol — may even reduce Streptococcus mutans, the primary bacteria behind tooth decay. For decades, dentists have recommended them to lower the risk of caries while still satisfying sweet cravings.

But new research suggests the story is more complex. A large Neurology study has found that frequent use of most popular low- and no-calorie sweeteners may accelerate cognitive decline, raising new questions about their long-term impact on overall health.

Study links artificial sweeteners to faster memory decline

The research, led by Dr. Claudia Kimie Suemoto at the University of São Paulo, followed 12,772 adults in Brazil over eight years, with an average age of 52. Participants were grouped by how much they consumed artificial sweeteners such as aspartame, saccharin, acesulfame-K, erythritol, xylitol, sorbitol, and tagatose.

Key findings include:

  • People with the highest intake (≈191 mg/day) experienced a 62% faster decline in memory and thinking skills compared to low-consumption groups (≈20 mg/day).
  • This cognitive decline equated to an additional 1.6 years of brain aging.
  • The link was strongest in participants under 60 years old and in those with diabetes.
  • Only tagatose showed no association with memory decline.

“These findings suggest that while sugar substitutes may support oral health, they could carry risks for long-term brain health, especially when consumed heavily,” said Dr. Suemoto.

Balancing oral and brain health

From a dental perspective, sugar substitutes remain valuable tools for cavity prevention. A 2023 systematic review in ScienceDirect confirmed that sugar alcohols such as xylitol, sorbitol, erythritol, and maltitol consistently reduce cavity-causing bacteria in saliva and dental plaque.

High-intensity sweeteners like aspartame, sucralose, saccharin, and stevia are considered non-cariogenic, meaning they do not contribute to cavities. However, evidence is limited on whether they actively improve oral health beyond reducing sugar intake.

This means that for protecting teeth and gums, artificial sweeteners are still better than sugar. The concern, however, is their potential trade-off in brain health when consumed in large amounts over time.

What dentists and health experts recommend

Experts stress that the study shows correlation, not causation. Still, the findings are significant enough to encourage moderation.

Practical tips include:

  • Check product labels for hidden sweeteners in drinks, snacks, and processed foods.
  • Prioritize natural sweetness from fruits where possible.
  • Use xylitol wisely — for cavity protection, especially in sugar-free gums and mints, but avoid overconsumption.
  • Limit long-term heavy use of aspartame, saccharin, and other artificial sweeteners until further research clarifies brain health impacts.
